BBC halts phone-in contests

A scandal erupts in relation to the use of premium-rate phone quizzes in conjunction with various forms of broadcast media. In response to this. The BBC suspends its premium rate interactive services whilst an "internal audit" takes place. The audit gives the all-clear and the contests are resumed again.

Then another investigation reveals a whole list of "editorial breaches" in respect of phone-ins and interactive competitions across a whole range of BBC programming. As a result, the BBC suspends all of its premium rate interactive services until further investigations have been completed.

So what exactly did happen the first time round?
